Smile Wrong Fork?

What vehicle is this clutch fork from?

It’s seems like it’s designed to pivot on a ball but my bellhousing only has a thin piece of flat metal for it to pivot on…

I know exactly what it came out of. I have one myself. They are used in '80's trucks.
I had to replace that ball not too long after I got the truck. It only had around 52,000 on it. It's got about 178,000 on it now, so the ball has held up good this time around. The ball has a thread stud that screws into the bell and the wrench flat is about 1/4" wide if it's that wide. Then it necks down really small where the steel part of the ball is made to the wrench flat. This is where I expected it to break, mine actually broke where the threded stud meets the wrench flat. No here's the kicker, the ball is only half steel. The upper half is white hard plastic that rides against your fork.

I'm surprised though, the new one has lasted a long time compared to the original one. You'll need a truck bell in order to use it on anything.
