You will most likely have to cut it yourself using an original as a template. Plastic supply houses would have the best prices for lexan. (Cut out the middle-man.)
I used 3/16" for the windshield and 1/4" elsewhere when I ran a Challenger in brackets.

They have different versions, some of which are more scratch-resistant. Isoprophyl alcohol is good for removing shoe polish as it literally melts it off. Unless you buy the special stuff to write on your windows. A good plastic spray-on cleaner/polish, which should also be available at a plastic supplier will help prolong the life/visibility of the lexan.

Have you tried TAP Plastics in San Jose on Alameda @ Race St.?

They do almost anything custom. Warning, they are pricey.

Hey thanks for the replies. For you guys that got custom work done, did you need to leave them your car/windshield to use as a mold, or did they already have a template to work with?

I think there's even a TAP plastics here in downtown Mountain View. Thanks.

At TAP, you will almost definitely have to leave your windshield as a template. I haven't had windshields done there, but some other things and they don't keep any templates of anything there.
